چکیده

Troponins are recommended as the biomarkers of choice for the detection of myocardial infarction (MI) by the Universal Definition.1 This is well accepted for the diagnosis of type 1 or spontaneous MI, in which any elevation in the setting of ischemia is required but not for the diagnosis of type 4a or MI associated with percutaneous coronary intervention (PCI), in which an isolated 3 the upper reference limit elevation is required. Creatine kinase MB (CKMB) is widely favored because a number of studies support a relationship between CKMB elevation after PCI and longterm mortality with the cut-point appearing to be about 8 to 10 elevation of CKMB.2–5

The prequel: defining prognostically important criteria in the periprocedural PCI troponin saga.

Rise of serum troponin levels following uncomplicated elective percutaneous coronary interventions in patients without clinical and procedural signs suggestive of myocardial necrosis.

INTRODUCTION The new definition of periprocedural myocardial infarction (type 4a MI) excludes patients without angina and electrocardiographic or echocardiographic changes suggestive of myocardial ischemia even though significant serum troponin elevations occur following percutaneous coronary intervention (PCI). AIM To evaluate the incidence and predictors of serum troponin rise following ele...

Predictors of coronary intervention-related myocardial infarction in stable angina patients pre-treated with statins

INTRODUCTION Peri-procedural myocardial infarction (PMI) is a frequent and prognostically important complication of percutaneous coronary intervention (PCI). This study was designed to determine the predictors of PMI in patients pre-treated with statins. MATERIAL AND METHODS A total of 418 stable angina pectoris patients taking statins and aspirin were included. All the patients underwent PCI...

Predictors of periprocedural (type IVa) myocardial infarction, as assessed by frequency-domain optical coherence tomography.

BACKGROUND Frequency-domain optical coherence tomography (FD-OCT) is easily able to define both pre- and post-stenting features of the atherosclerotic plaque that can potentially be related to periprocedural complications. We sought to examine which FD-OCT-defined characteristics, assessed both before and after stent deployment, predicted periprocedural (type IVa) myocardial infarction (MI). ...

Coronary plaque morphology on multi-modality imagining and periprocedural myocardial infarction after percutaneous coronary intervention☆

Percutaneous coronary intervention (PCI) may be complicated by periprocedural myocardial infarction (PMI) as manifested by elevated cardiac biomarkers such as creatine kinase (CK)-MB or troponin T. The occurrence of PMI has been shown to be associated with worse short- and long-term clinical outcome. However, recent studies suggest that PMI defined by biomarker levels alone is a marker of ather...
